Hospice Registered Nurse (RN) Case Manager in Lakeview, Kenner, Metairie, and Chalmette, Louisiana At Interim HealthCare Hospice, our mission is to provide compassionate, patient-centered care during life’s most meaningful and vulnerable moments. • Competitive Compensation -Salary range: $80,000–$88,000 annually, with flexibility for the right candidate
• Consistent & Predictable Scheduling: Full-time: Monday–Friday, 8:00 AM–5:00 PM. On-call responsibilities as needed. PRN RN opportunities also available

What You’ll Do as a Hospice RN Case Manager:

• Complete holistic assessments addressing physical, emotional, psychosocial, and spiritual needs
• Develop individualized care plans that honor each patient’s goals
• Collaborate with an interdisciplinary team including physicians, social workers, chaplains, CNAs, and volunteers
• Provide education to patients and families on symptom management, medications, and the hospice journey
• Deliver compassionate end-of-life care and comfort measures
• Maintain accurate documentation in accordance with organizational and regulatory standards
• Participate in quality initiatives that elevate hospice care for the community

What We’re Looking For:

• Graduate of an accredited nursing program with an active Louisiana RN license
• At least 1 year of hospice nursing experience
• Current CPR certification
• Valid driver’s license, auto insurance, and reliable transportation
• Strong clinical assessment skills with a compassionate, patient-centered approach
• Knowledge of state and federal hospice/home health regulations
