Pride Health is excited to partner with a leading health system in Muskogee, OK , seeking a compassionate and motivated Social Worker (MSW) to join their interdisciplinary care team. ✨ Role Highlights

  • Schedule: Monday–Friday, 8:00 AM–4:30 PM (Day shift)
  • Employment Type: Full-time, permanent
  • Location: Muskogee, Oklahoma
  • Compensation: $29–$40/hr based on experience
  • Bonus: $5,000 Sign-On Bonus
  • Setting: Hospice (hospital, community, and in-office)

💼 What You’ll Do

  • Provide skilled clinical interventions to support patients and families through crisis, illness, abuse, end-of-life concerns, and life-changing events.
  • Lead comprehensive case management , discharge planning, care coordination, and psychosocial assessments.
  • Collaborate with a diverse interdisciplinary team to build complex discharge plans and ensure seamless transitions of care.
  • Utilize hospital and community resources to support patient needs — including insurance coordination and community service referrals.
  • Document care plans, interventions, and outcomes clearly and completely in the EMR.
  • Participate in community education , support groups, and ongoing professional development.

🎯 What We’re Looking For

  • Master of Social Work (MSW) or closely related degree from an accredited program – required
  • Experience working with hospital, community health, or hospice populations (required)
  • Ability to work directly with patients, families, and providers in fast-paced settings
  • Strong communication, problem-solving, and crisis-intervention skills
  • Computer literacy required
  • Preferred: LCSW or LPC licensure & bilingual fluency
