Key Duties - Provide individualized nutrition assessment, diagnostic therapy and counseling services for the purpose of diabetes self-management for patients and caregivers
- Screen, assess, monitor, and provide recommendations for both in-patients and outpatients
- Collaborate with the interdisciplinary team in meetings and/or rounds
- Develops and provides programming, education, and resources for the population
- Experience with electronic charting, the Nutrition Care Process and writing PES statements to provide appropriate documentation for the plan of care
- Demonstrates accountability for the proper use of patients' protected health information
- Ability to perform work independently with minimal training
Required Licenses/Certifications - Current Registered Dietitian credentialed by CDR
- GA state dietitian license is required prior to starting position
- CDE/CDCES strongly preferred
