Job Title: RN / LPN – Quality Assurance Nurse

Location: Flagstaff, AZ (Onsite – Local candidates only)

Duration: 12/15/2025 – 06/30/2026

Schedule: 8 hrs/day, Monday–Friday (varied weekends if required)

Shift Options: 7am–3pm | 8am–4pm | 8am–5pm

Role Summary

The Quality Assurance Nurse will support Quality Assurance (QA), Staff Education, and Infection Prevention functions. This role ensures compliance with state, federal, and VA regulations through audits, reporting, training, and infection surveillance.

Key Responsibilities

• Implement and monitor quality assurance policies using nursing home regulations and evidence-based practices
• Conduct audits, interviews, and observations across departments to ensure compliance
• Review CASPER reports and develop QAPI-based Performance Improvement Plans (PIPs)
• Prepare and present QA reports, data summaries, and compliance updates
• Assist leadership with corrective action plans and deficiency resolution
• Coordinate facility preparation for state and federal surveys
• Chair Quality Assurance and Infection Control Committee meetings
• Investigate incidents, abuse, neglect, and non-compliance issues
• Develop and deliver staff training and compliance education programs
• Track and trend infections using McGeer and non-McGeer criteria
• Perform daily infection control surveillance rounds
• Maintain compliance documentation and training records
• Stay current with CMS, CDC, and regulatory updates

Required Qualifications

• Active Arizona RN or LPN License
• Infection Preventionist Certification
• DPS Level 1 Fingerprint Clearance Card
• 2-Step TB Test (clear/negative)
• CPR/BLS Certification
• Valid Arizona Driver’s License
• Ability to start within 2 weeks of offer
• Must be local to Flagstaff, AZ at time of submission
