Location: Rochester, NY - Hybrid, mix of telehealth and home visits
Type: Direct hire, full time offer with client!
Schedule: Monday through Friday, 8am to 5pm
Pay: $73,500 to $77,500 yearly
Responsibilities of the RN Care Manager:
Qualifications for the RN Care Manager:
· RN license in NY, active and in good standing
· Two years of nursing experience, one year in pediatrics, OB/GYN or community/public health
· Proficiency with EHR and data documentation
· BLS through the American Heart Association
· NYS Infection Control certification
