Licensed Master Social Worker (LMSW)

📍 Location: New York, NY

🕑 Schedule: Monday – Friday, 1:00 PM-9:00 PM

📅 Contract : 13 weeks

💰 Compensation: $55 – $60 per hour (based on experience)

Job Summary:

Pride Health is seeking a Licensed Master Social Worker (LMSW) to join our team in New York, NY. The LMSW will provide telephonic and face-to-face care management services to members at designated healthcare facilities, focusing on HIV-positive, transgender, and homeless populations. This role supports patient-centered care through comprehensive case management, care coordination, and health education to improve quality of life and health outcomes.

Key Responsibilities:

• Provide care management and coordination services, including transition of care, follow-up, and ancillary support.
• Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams.
• Facilitate access to quality healthcare services for members with complex health needs.
• Offer education and health coaching related to HIV disease management, risk reduction, and chronic comorbidities.
• Communicate regularly with providers, case managers, and agencies to ensure continuity of care.
• Identify member needs and trends using encounter, pharmacy, and assessment data.
• Document all care coordination activities in electronic health records following regulatory standards.
• Mentor and support Health & Wellness Advisors and participate in case conferences and team meetings.
• Assist with audits, quality improvement initiatives, and departmental projects as needed.
• Maintain compliance with federal, state, and organizational policies.

Skills & Qualifications:

• Minimum of 1 year of LMSW experience (Required).
• Strong communication and care coordination skills.
• Experience with HIV, chronic illness, or community-based populations preferred.
• Ability to manage multiple cases while maintaining accuracy and compassion.
• Proficient in electronic documentation and data tracking.

Education & Certification:

• Master’s Degree in Social Work (MSW) from an accredited institution (Required).
• Valid LMSW License issued by the New York State Education Department (Required).
• BLS Certification (Preferred).
